Hair Loss (Alopecia)

Chemotherapy-induced alopecia (CIA) affects up to 65% of patients and is one of the most emotionally distressing side effects. Most hair loss is temporary — hair typically begins to regrow 3–6 months after chemotherapy ends. Some targeted therapies and hormone therapies cause thinning rather than complete loss. Scalp cooling can prevent hair loss in some cases.

Herbs & Supplements — Safety Information

Herbal information is for educational purposes. Many herbs interact with chemotherapy and other medications — consult your oncologist before use.

When to Seek Medical Help Immediately

  • Scalp infections, sores, or significant scalp pain
  • Hair loss not recovering 6+ months after treatment ends
  • Ask your oncologist about scalp cooling (Paxman, DigniCap) before starting chemotherapy

4 Natural Remedies

🧊

Scalp Cooling (Cold Cap)

Best for: Prevention of chemotherapy-induced hair loss — must start from first infusion

Strong Evidence

Scalp cooling (cold caps or scalp cooling systems) during chemotherapy infusions constricts blood vessels in the scalp, reducing drug concentration reaching hair follicles. Clinical trials show it preserves hair in 50–65% of breast cancer patients on taxane chemotherapy. It is available in many oncology centres.

🧪 How to Prepare

Ask your oncology team about scalp cooling availability before your first infusion. Systems include Paxman and DigniCap (hospital-grade) or manual ice caps. The cap is worn 30 minutes before, throughout, and 30–90 minutes after each infusion.

⏰ When to Take

Must be started from the first chemotherapy infusion — not effective if started mid-treatment.

Rosemary Oil Scalp Massage

Best for: Promoting regrowth after treatment, reducing shedding

Moderate Evidence

A randomised controlled trial published in 2015 compared rosemary oil vs minoxidil 2% (the standard pharmaceutical hair loss treatment) — rosemary oil performed equally well at 6 months with less scalp itching. Rosemary contains carnosic acid which promotes nerve growth and blood circulation to hair follicles.

🧪 How to Prepare

Mix 10 drops of rosemary essential oil in 1 tablespoon of carrier oil (jojoba, castor, or coconut oil). Massage into the scalp for 5–10 minutes. Leave on for 30–60 minutes then wash out. Alternatively, add rosemary oil to your shampoo (5 drops per tablespoon of shampoo).

⏰ When to Take

3–5 times per week, ideally once regrowth phase begins (post-treatment). Can also be used during treatment to maintain follicle health.

Active compound: Carnosic Acid, 1,8-cineole, Ursolic Acid

Castor Oil Scalp Treatment

Best for: Post-treatment hair regrowth support, moisturising dry scalp

Traditional Use

Castor oil is rich in ricinoleic acid, which has anti-inflammatory properties and has been shown to increase prostaglandin E2 — a compound that stimulates hair follicle activity. It also creates a protective, moisturising coating on fragile regrowing hair.

🧪 How to Prepare

Warm 1–2 tablespoons of cold-pressed castor oil slightly (place jar in warm water for 5 minutes). Apply to scalp and hair roots using fingertips or a dropper. Massage in gently for 5 minutes. Cover with a shower cap and leave for 1–2 hours (or overnight). Wash out thoroughly.

⏰ When to Take

Once or twice weekly. Castor oil is thick — mix with lighter oil (jojoba, argan) if difficult to wash out.

Active compound: Ricinoleic Acid

Biotin & Collagen Supplementation

Best for: Post-treatment hair regrowth, hair thinning during hormone therapy

Moderate Evidence

Biotin (B7) is essential for keratin production — the protein that makes up hair. Collagen (specifically marine or Type I collagen) provides amino acids (especially proline and glycine) that are building blocks for hair follicle structure. Though evidence for cancer-specific hair loss is limited, both are safe and widely used in hair restoration.

🧪 How to Prepare

Biotin: 2500–5000mcg daily. Marine collagen: 10–15g daily (as powder in a smoothie or liquid). Food sources of biotin: eggs (especially yolk), nuts, sweet potatoes. Food sources of collagen precursors: bone broth, meat.

⏰ When to Take

Daily with breakfast. Collagen powder in a morning smoothie. Allow 3–6 months before judging effectiveness.
