Red Clover / Rotklee
Trifolium pratense
Red Clover contains the highest concentration of isoflavones of any plant — particularly formononetin, biochanin A, daidzein, and genistein. These phytoestrogens bind oestrogen receptors with weak agonist/antagonist effects, showing complex anti-cancer activity that varies by cancer type and hormonal status. In breast cancer, red clover isoflavones competitively inhibit oestrogen receptor binding (protective in ER+ cancers when used correctly). In prostate cancer, biochanin A and formononetin inhibit 5-alpha reductase, reduce PSA, and induce apoptosis in LNCaP cells. Multiple RCTs show red clover significantly reduces hot flushes in breast cancer survivors on tamoxifen — a major quality-of-life benefit. Biochanin A is particularly potent: it inhibits CYP1B1 (an enzyme that activates carcinogens in breast tissue) and blocks the P-glycoprotein drug efflux pump that causes multidrug resistance.

Preparations
Standardised Isoflavone Capsules (Promensil)
40–80 mg standardised red clover isoflavones daily (Promensil® or equivalent). Take with food. Do not exceed 160 mg daily.
Red Clover Tea
2–3 tsp dried red clover flowers steeped in boiling water for 10 minutes. Drink 1–2 cups daily. Lower isoflavone concentration than supplements but pleasant and well-tolerated.

Cautions & Interactions
- AVOID in active hormone-receptor-positive breast cancer without oncologist approval — isoflavones are phytoestrogens
- Do not use with tamoxifen or aromatase inhibitors without specialist guidance — interaction risk is debated
- Avoid in pregnancy and breastfeeding
- May interact with anticoagulants — contains coumarins
- Evidence is mixed for ER+ breast cancer — the safest use is post-treatment for hot flush management
